PRE-PARTICIPATION PHYSICAL FORM
Only the UIL Physical forms will be accepted. A copy can be printed from the links below or from the athletic training staff. The physical form includes the Medical History page. A parent and student signature is required on this page.
Physical forms can be turned in at the Kyle G. West Field House Office (next to Hutto Memorial Stadium) before or after school for Grades 10-12. For Grades 6-9, forms can be turned in at the Hutto Ninth Grade Center before or after school. If you have any additional questions, please email us at athletictrainers@huttoisd.net.
