SPORTS PHYSICALS & MEDICAL FORMS
Before participating in Hutto ISD Athletics, student-athletes must have all required UIL medical documentation completed and on file.
This page will help families understand the sports physical process, download the correct UIL form, and know where to submit completed paperwork.
IMPORTANT: Do not wait until the first day of practice to complete your student's physical requirements. Missing, incomplete, or unsigned paperwork may delay athletic participation.
DOES MY STUDENT NEED A PHYSICAL?
Hutto ISD requires all athletes to obtain a new physical every school year. The physical must be dated after May 01 to be accepted for the next school year.
If you are unsure whether your student needs a new physical for the 2026–27 school year, please contact the Hutto ISD Athletic Training Staff before scheduling an appointment.
PHYSICAL EXAM VS MEDICAL HISTORY FORM
These are two important parts of the UIL medical clearance process.
PHYSICAL EXAM
When a physical is required, the examination must be completed by an approved medical professional.
The provider will complete the Physical Examination portion of the UIL form and determine whether the student is medically cleared for athletic participation. The provider must mark cleared, not cleared, or needs further evaluation. The provider must also sign and date the physical.
Physicals cannot be accepted prior to May 01 for the new school year.
MEDICAL HISTORY FORM
The Medical History Form is completed every year and is included with the UIL physical paperwork.
The student and parent/guardian must:
- Review all medical-history questions
- Answer all questions completely
- Provide any requested explanations
- The student and Parent/Guardian must sign the page in the designated box at the bottom of the form.
Certain medical-history responses may require additional evaluation or medical clearance before participation.
COMPLETE YOUR PHYSICAL IN 4 EASY STEPS
1. DOWNLOAD THE CURRENT UIL PHYSICAL FORM
Hutto ISD Athletics will only accept the official UIL Pre-Participation Physical Evaluation form.
2. COMPLETE THE MEDICAL HISTORY
Parents/guardians and students should complete the Medical History portion of the form and provide all required signatures.
We recommend completing this section before arriving for the physical appointment.
3. COMPLETE THE PHYSICAL EXAMINATION
Take the UIL form with you to your student's physical appointment.
UIL physical examinations may be completed, signed, and dated by an approved:
- Physician
- Physician Assistant
- Advanced Practice Registered Nurse
- Doctor of Chiropractic
The medical provider must complete, sign, and date the appropriate Physical Examination section of the UIL form.
4. SUBMIT THE COMPLETED FORM
After the physical is completed, review the paperwork carefully before submitting it to Hutto ISD Athletics.
All student athletes are required to obtain a new UIL Athletic Physical each school year. We CANNOT accept physicals for the new school year dated prior to May 01
BEFORE YOU SUBMIT YOUR FORM
Please make sure the following items have been completed:
Current UIL Physical Examination form was used
Medical History section is complete
All medical-history questions have been answered
Parent/guardian signature is included
Medical provider completed the examination
Medical provider indicated the student's clearance status
Medical provider signed and dated the form
All required pages are included
Incomplete forms may delay athletic participation.
